Newton Falls Schools Registration Now Through Final Forms

The Newton Falls Exempted Village School District is excited to have partnered with Final Forms for all registration and documentation needed by the school district for your children. Please connect to Final Forms for registration by clicking on the image below.

To Register a Student in the District

To begin your child's journey as a Tiger, they first need to be registered within the school district through Final Forms (link above)

The Newton Falls Exempted Village School District is required by Ohio Revised Code to keep permanent records on each registered student within the boundaries of the school district. The following documents will be required to complete the full registration of your child.

• Proof of Birth (Birth Certificate or equivalent) of the child
• Immunization Records of the child
• Proof of residency within the boundaries of the Newton Falls Exempted Village School District
• Copy of the parent's driver's license (or identification card, office will make a copy)
• Copy of custody/court papers when:
  • Child is not living with the biological mother at her residence shown in proof of residency
  • The Biological mother and father were never married, court document that shows that the father has parental rights (if that is wanted)
  • Any applicable court documents showing temporary custody of child, other than the biological mother
  • If child is living with grandparents whom reside within the school district

Scheduling a Registration Appointment

Once you have set up your Final Forms account and everything is filled out, you can schedule a finalization registration appointment with the district. Appointments are scheduled Monday through Friday on open school days during the office hours of the Newton Falls High School. ALL STUDENTS K-12 will be originally registered through the High School Office.

The appointment should only take ten to fifteen minutes to finalize. Please be certain to bring with you a photo identification and your child(s) birth certificate (or equivalent). The district will make copies of these and attach them to your registration.

Any additional documentation that you were not able to upload should be brought with you as well such as:

• Any court or custody documents
• Proof of residency
• Affidavit of Shared Residency Form (if applicable)
• Immunization Records of the child
• Parent's Driver’s License
• Proof of Birth (Birth Certificate or equivalent)
